Dues, Assessments, & Insurance Forms
Pilots at U.S.-based carriers can authorize payroll deductions for dues checkoff (DCO), assessments checkoff (ACO), and insurance checkoff (ICO) by completing the applicable form below.
To submit a checkoff form, download the appropriate form first (forms cannot be submitted through most web browsers). You can email, fax, or mail the completed form to ALPA using the instructions provided.
